Цель – оценить влияние противоревматической терапии, которая проводится в соответствии с принципом стратегии «Лечение до достижения цели», на прогрессирование атеросклероза и КВР у больных ранним РА. Материал и методы. В исследование включено 74 пациента (72% женщин, медиана возраста – 56 лет) с ранним РА с умеренной и высокой активностью (медиана DAS28 – 5,6), не получавших ранее БПВП и глюкокортикоиды (ГК). Все больные были позитивны по антителам к циклическому цитруллинированному пептиду и 87% больных – по ревматоидному фактору. Всем пациентам начата терапия метотрексатом (МТ) с эскалацией дозы до 25–30 мг/нед подкожно, при отсутствии эффекта через 3 мес к этому лечению присоединяли генно-инженерные биологические препараты (ГИБП). Через 6 мес 39% больных достигли ремиссии, 19% – низкой активности, 35 и 7% сохранили соответственно умеренную и высокую активность РА. Большинство (69%, n=20) пациентов, достигших ремиссии, находились на монотерапии МТ, 9 (31%) – получали комбинированную терапию МТ+ГИБП, тогда как среди пациентов, не достигших ремиссии, 15 (33%) находились на монотерапии МТ, 30 (67%) получали комбинированное лечение МТ+ГИБП. Оценка традиционных факторов риска сердечно-сосудистых заболеваний (ССЗ) с определением суммарного коронарного риска по шкале SCORE, в том числе в модификации EULAR (mSCORE), атеросклероза сонных артерий (АСА) по данным дуплексного сканирования, кальциноза коронарных артерий (ККА) по данным мультиспиральной компьютерной томографии и оценка степени КВР проведены всем пациентам с ранним РА до начала терапии и после 6 мес лечения. Результаты и обсуждение. Частота артериальной гипертензии, избыточной массы тела, абдоминального ожирения, гиподинамии, курения, сахарного диабета 2-го типа через 6 мес достоверно не изменилась. Отмечено увеличение уровня общего холестерина (ХС) на 7% (p&lt;0,05), ХС липопротеидов низкой плотности на 9% (р&lt;0,01), ХС липопротеидов высокой плотности на 26% (р&lt;0,005), индекса массы тела (ИМТ) на 1% (р&lt;0,01), снижение индекса атерогенности (р&lt;0,005). Изменение концентрации показателей липидного спектра крови положительно коррелировало с динамикой ИМТ (р&lt;0,05) и отрицательно – с динамикой уровней маркеров воспаления (С-реактивного белка, СОЭ; р&lt;0,05). Через 6 мес отмечено нарастание суммарного КВР по шкале SCORE и mSCORE (р&lt;0,005). Увеличение частоты АСА с 59 до 72% и ККА с 42 до 47% привело к нарастанию доли лиц с очень высоким КВР с 67 до 76%, однако различия не достигли статистической значимости. В группах больных, получающих монотерапию МТ и МТ+ГИБП, отмечено сопоставимое нарастание частоты АСА и ККА. У больных, не достигших ремиссии РА, увеличение частоты АСА было на 18% больше, чем у тех, кто ее достиг (3%; р=0,05). Значимое прогрессирование АСА отмечено в группе лиц, не достигших ремиссии и не получавших статины (р=0,05), в то время как среди лиц, получавших статины и достигших ремиссии РА, частота АСА осталась неизменной. Таким образом, по предварительным данным исследования РЕМАРКА, замедление прогрессирования атеросклероза у больных ранним РА возможно в случае достижения ремиссии заболевания на фоне противоревматической терапии при одновременном регулярном использовании статинов, что в дальнейшем может ассоциироваться со снижением КВР. </p></abstract><trans-abstract xml:lang="en"><p>Most patients with early rheumatoid arthritis (RA) have a high or very high cardiovascular risk (CVR) before therapy with disease-modifying antirheumatic drugs (DMARDs). Objective: to evaluate the impact of antirheumatic therapy performed in accordance with the Treat-to-Target strategy on the progression of atherosclerosis and CVR in patients with early RA. Subjects and methods. This investigation enrolled 74 patients (72% women; median age, 56 years) with early RA having moderate to high activity (median DAS28, 5.6) who had not previously received DMARDs and glucocorticoids (GCs). All patients were anticyclic citrullinated peptide antibody-positive and 87% of the patients were rheumatoid factor-positive. All patients received methotrexate (MT) subcutaneously with dose escalation up to 25–30 mg/week, in case of its inefficiency at 3 months a biological agent (BA) was added. After 6 months, 39% of the patients achieved remission; 19% had low; 35 and 7% had moderate and high disease activity, respectively. The majority (n = 20 (69%)) who achieved remission received MT monotherapy; 9 (31%) – MT + BA whereas among the patients who did not achieve remission 15 (33%) and 30 (67%) respectively. At baseline and after 6 months of treatment, traditional CVR factors were assessed in all patients, by determining the total coronary risk by the SCORE scale, including that modified by EULAR (mSCORE), carotid artery atherosclerosis (CAA) by duplex scanning data, coronary calcification (CC) by multislice spiral computed tomography and by estimating the degree of CVR.Results and discussion. The rates of hypertension, overweight, abdominal obesity, low activity, smoking, and type 2 diabetes mellitus did not change significantly after 6 months. There were increases in the levels of total cholesterol by 7% (p &lt; 0.05), low-density lipoprotein cholesterol by 9% (p&lt;0.01), high-density lipoprotein cholesterol by 26% (p &lt; 0.005), and body mass index (BMI) by 1% (p &lt; 0.01) and a decrease in the atherogenic index (p&lt;0.005). The change in blood lipid spectrum concentrations was correlated positively with trends in BMI (p &lt; 0.05) and negatively with those in the levels of inflammatory markers (C-reactive protein, erythrocyte sedimentation rate; p &lt; 0.05). Following 6 months, there was a rise in the total CVR according to the SCORE and mSCORE scales (p &lt; 0.005). The elevations in the rates of CAA from 59 to 72% and CC from 42 to 47% resulted in an increase in the proportion of persons with very high CVR from 67 to 76%; however, the differences failed to attain statistical significance. There was a similar increase in the rates of CAA and CC in the groups of patients receiving monotherapy with MT and MT + BA. In the patients who failed to achieve remission in RA, the rise in the rate of CAA was 18% higher than in those who did (3%) (p = 0.05).
