Наступление беременности у них часто сопровождается снижением активности РА, а после родов может возникать  обострение. Регулярный мониторинг активности заболевания в период гестации и после родов является  необходимым  условием адекватной  коррекции терапии у данной  категории больных.</p><p>Цель – определить оптимальный метод оценки  активности РА во время беременности и после родов.</p><sec><title>Материал и методы</title><p>Материал и методы. Проспективно в каждом триместре гестации и в течение 12 мес после родов прослежено 32 беременности у 29 женщин  с достоверным  РА (критерии ACR, 1987), обследованных в ФГБНУ  НИИР им. В.А. Насоновой в период с февраля2011 г. по август2014 г.</p></sec><sec><title>Результаты</title><p>Результаты. При сравнении разных методов оценки  активности РА установлено, что DAS28-СОЭ  показывает завышенные результаты в связи с физиологическим повышением СОЭ во время беременности. На индексы CDAI и SDAI большое влияние  оказывает  субъективная  оценка состояния больной своего здоровья, которая может завышаться  во время беременности и в первый месяц после родов. Индекс  DAS28-СРБ(3), рекомендованный  в международной литературе для оценки  активности РА у беременных, показал идентичную  динамику с DAS28-СРБ(4). Последний имеет широкое  применение в международных  исследованиях, валидирован на большом числе больных. Таким образом,  для мониторинга активности РА во время беременности и после родов оптимальным можно считать индекс DAS28-СРБ(4).</p></sec></abstract><trans-abstract xml:lang="en"><p>Rheumatoid arthritis (RA) is a chronic disease that may affect women of childbearing age. The occurrence  of their pregnancy is frequently accompanied by the lower activity of RA and its exacerbation may occur postpartum.  Regular disease activity monitoring  during pregnancy and postpartum  is a necessary condition  for adequate therapy correction in this category of patients.</p><sec><title>Objective</title><p>Objective: to determine an optimal method to assess RA activity during pregnancy and postpartum</p></sec><sec><title>Subjects and methods</title><p>Subjects and methods. Thirty-two  pregnancies were prospectively followed up during each trimester and within 12 months postpartum  in 29 women with RA (according  to the 1987 ACR criteria) who had been examined at the V.A. Nasonova Research Institute  of Rheumatology  from February 2011 to August 2014.</p></sec><sec><title>Results</title><p>Results. Comparison  of different methods to assess RA activity demonstrated that DAS28-ESR  shows overrated estimates due to a physiological ESR elevation during pregnancy. CDAI and SDAI are greatly affected by a patient's subjective assessment of his/her  health, which may be overestimated during pregnancy and in the first month after giving birth. DAS28-CRP(3) recommended in the world literature to assess RA activity in pregnant women showed the same changes as DAS28-CRP(4). The latter  is widespread in international studies and has been validated in a large number of patients.
