В последние годы были расшифрованы многообразные нарушения иммунитета, лежащие в основе аутоиммунных ревматических заболеваний (АРЗ), выявлена связь между развитием АРЗ и аутовоспалительных заболеваний и синдромов, разработана классификация иммуновоспалительных заболеваний человека. В статье рассматриваются результаты собственных исследований, касающихся лечения АРЗ с использованием инновационных генно-инженерных биологических препаратов, изучения патогенетических механизмов и диагностики АРЗ на основе иммунологических и молекулярно-биологических методов исследования широкого спектра молекулярных и клеточных биомаркеров (аутоантитела, острофазовые белки воспаления, цитокины, хемокины, маркеры активации сосудистого эндотелия, компоненты системы комплемента, субпопуляции лимфоцитов, продукты метаболизма костной и хрящевой ткани, генетические, эпигенетические, транскриптомные маркеры), подходов к персонифицированной терапии АРЗ.</p></abstract><trans-abstract xml:lang="en"><p>According to the present-day views, autoimmunity is a complex pathological process, the essence of which is intolerance and hence a pathological immune response to intrinsic tissue components (autoantigens), which underlies the pathogenesis of a broad spectrum of human autoimmune diseases. Recently, diverse immune disorders underlying autoimmune rheumatic diseases (ARD) and syndromes have been revealed; an association has been found between the development of ARD and autoinflammatory diseases and syndromes; a classification of human immunoinflammatory diseases has been elaborated. The paper considers the results of the authors’ investigations of ARD treatment with innovative biologics, the pathogenetic mechanisms and diagnosis of ARD, by conducting immunological and molecular biological studies of a wide range of molecular and cellular biomarkers (autoantibodies, acute phase proteins, cytokines, chemokines, vascular endothelial activation markers, complement system components, lymphocyte subpopulations, bone and cartilage tissue metabolic products, genetic, epigenetic, transcriptomic markers), and approaches to personalized treatment of ARD.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>аутоиммунные ревматические заболевания</kwd><kwd>биомаркеры</kwd><kwd>аутоантитела</kwd><kwd>цитокины</kwd><kwd>генно- инженерные биологические препараты</kwd></kwd-group><kwd-group xml:lang="en"><kwd>autoimmune rheumatic diseases</kwd><kwd>biomarkers</kwd><kwd>autoantibodies</kwd><kwd>cytokines</kwd><kwd>biological agents</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">McGonagle D, McDermott MF.
