В то же время дифференциально-диагностическое значение этого признака на ранней стадии подагрического артрита (ПА) изучено недостаточно.</p><p>Цель исследования – оценить специфичность и чувствительность факта обнаружения линейных гиперэхогенных включений в дифференциальной диагностике раннего ПА, в сравнении с повышением уровня мочевой кислоты, наличием подкожных тофусов и определением внутрикостных кист при рентгенографии.</p><sec><title>Материал и методы</title><p>Материал и методы. Проведен анализ данных обследования 119 больных артритом с длительностью симптомов до 6 мес на этапе дифференциальной диагностики. В дальнейшем были установлены следующие диагнозы: ПА – у 32 больных, остеоартрит (ОА) – у 28, ревматоидный артрит (РА) – у 28, псориатический артрит (ПсА) – у 16. Наряду с клиническим, лабораторным и рентгенологическим обследованиями всем больным проводилась ультразвуковое исследование (УЗИ) воспаленных суставов линейным датчиком (частота – 18 МГц) с целью выявления линейных гиперэхогенных включений в гиалиновом хряще (уратные депозиты).</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Уратные депозиты по данным УЗИ суставов при ПА были выявлены у 28 (87,5%) больных, при ОА – у 3 (10,7%) и при ПсА – у 1 (6,3%). Чувствительность остальных признаков подагры на ранней стадии ПА составила: 21,7% – для рентгенологического определения внутрикостных кист, 25,0% – для подкожных тофусов, 81,3% – для гиперурикемии. Специфичность ультрасонографического выявления уратных депозитов при ПА составила 94,4% и была сопоставима с таковой для остальных признаков подагры (86,1–98,6%).</p></sec><sec><title>Заключение</title><p>Заключение. Сонографическое выявление депозитов солей мочевой кислоты у больных подагрой характеризуется высокой чувствительностью и специфичностью в диагностике ПА в течение первых 6 мес от появления первых симптомов. </p></sec></abstract><trans-abstract xml:lang="en"><p>A sonographic sign, such as linear hyperechoic inclusions or uric acid deposits in the hyaline cartilage, has been recently introduced into the system of gout classification criteria. At the same time, the differential diagnostic value of this sign in early gouty arthritis (GA) has not been adequately explored.</p><sec><title>Objective</title><p>Objective: to evaluate specificity and sensitivity for detecting linear hyperechoic inclusions in the differential diagnosis of early GA in comparison with the increased level of uric acid, the presence of subcutaneous tophi, and the identification of intraosseous cysts at radiography.</p></sec><sec><title>Subjects and methods</title><p>Subjects and methods. The investigators analyzed the data of examining 119 patients with arthritis with its symptom duration within 6 months at the stage of differential diagnosis. The following diagnoses were further established: GA in 32 patients, osteoarthritis (OA) in 28, rheumatoid arthritis (RA) in 28, and psoriatic arthritis (PsA) in 16. Along with clinical, laboratory, and radiological examinations, all the patients underwent ultrasonography (USG) of inflamed joints with an 18-MHz linear transducer to identify linear hyperechoic inclusions (urate deposits) in the hyaline cartilage.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Joint USG revealed urate deposits in 28 (87.5%) patients with GA, in 3 (10.7%) with OA, and in 1 (6.3%) with PsA. The sensitivity of the other signs of gout in early GA was 21.7% for intraosseous cysts, 25.0% for subcutaneous tophi, and 81.3% for hyperuricemia. The specificity of USG for urate deposits in GA was 94.4% and was comparable with that for the other signs of gout (86.1–98.6%). Conclusion.
