Особенности изменений СО следует учитывать при ранней диагностике РА и разработке комплексной терапии заболевания.</p><p>Цель исследования – изучить изменения СО при артроскопии у больных с различной длительностью РА и разным уровнем антител к циклическому цитруллинированному пептиду (АЦЦП) в крови.</p><sec><title>Материал и методы</title><p>Материал и методы. Артроскопия была выполнена 37 пациентам с РА, использовался артроскоп (Karl Storz GmbH, Германия) диаметром 2,4 мм с углом 30°. У 17 больных продолжительность РА была &lt;2 лет, у 20 – варьировала от 2 до 8 лет; у 15 пациентов уровень АЦЦП в крови не превышал 60 Ед/мл, у 22 он был более высоким. Изменения СО оценивались полуколичественным методом.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. У пациентов с длительностью РА &lt;2 лет по данным артроскопии преобладает гиперемия СО с усиленным сосудистым рисунком (р&lt;0,01) и выявляются отложения фибрина (р&lt;0,05), а при длительности РА &gt;2 лет преобладает ворсинчатая гиперплазия с образованием булавовидных ворсин (р&lt;0,05). Повышение уровня АЦЦП &gt;60 Ед/мл ассоциировались с преобладанием воспалительной гиперплазии (р&lt;0,01), гиперемии и СО с выраженным сосудистым рисунком (р&lt;0,05), а также с наличием паннуса (р&lt;0,01).</p></sec></abstract><trans-abstract xml:lang="en"><p>Exudative or proliferative processes accompanied by joint destruction may predominate in the synovial membrane (SM) in different stages of rheumatoid arthritis (RA). The features of SM changes should be considered in the early diagnosis of RA and in the development of combination treatment for this disease.</p><sec><title>Objective</title><p>Objective: to study arthroscopic SM changes in patients with different RA durations and different blood anti-cyclic citrullinated peptide (anti-CCP) antibody levels.</p></sec><sec><title>Subjects and methods</title><p>Subjects and methods. 37 patients with RA underwent arthroscopy with an arthroscope of a 2.4-mm diameter and 30° angle (Karl Storz GmbH, Germany). RA duration was &lt;2 years in 17 patients and ranged from 2 to 8 years in 20; the blood level of anti-CCP antibodies did not exceed 60 IU/ml in 15 patients and it was higher in 22. SM changes were assessed by a semiquantitative method.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Arthroscopy revealed a preponderance of SM hyperemia with an increased vascular pattern (p&lt;0.01) and fibrin deposits (p&lt;0.05) in patients with a RA duration of &lt; 2 years and that of villous hyperplasia with the formation of club-shaped villi (p&lt;0.05) in those with a RA duration of &gt;2 years.
