Основная причина развития хронической боли в плече – субакромиальный импичмент-синдром (синдром сдавления ротаторов плеча – ССРП). Эта патология сопровождается повреждением и воспалением сухожилий мышц, определяющих основные движения в плече: надостной, подостной, подлопаточной и малой круглой. В настоящем обзоре рассмотрены вопросы патофизиологии ССРП, клиническая картина этого заболевания, дифференциальная диагностика с другими болезнями и патологическими состояниями, сопровождающимися болями в плече. Для лечения ССРП используется широкий спектр медикаментозных средств: нестероидные противовоспалительные препараты, локальные инъекции глюкокортикоидов и гиалуроновой кислоты. В обзоре приводятся данные клинических исследований, оценивающих сравнительную эффективность этих препаратов. Особое место уделяется новой методике лечения ССРП – применению обогащенной тромбоцитами плазмы (ОТП). Приводятся последние данные о механизме действия ОТП, классификация препаратов ОТП, методика приготовления и данные клинических исследований, в которых оценивались эффективность и безопасность этого средства.</p></abstract><trans-abstract xml:lang="en"><p>Shoulder pain associated with periarticular soft tissue lesion is a common pathology and one of the most common reasons to visit rheumatologists and traumatologists. The main cause of chronic shoulder pain is subacromial impingement syndrome (shoulder rotator compression syndrome (SRCS)). This pathology is accompanied by injury and inflammation of the tendons of muscles (supraspinatus, infraspinatus, subscapularis, and teres minor ones), which determine main movements in the shoulder. This review deals with the pathophysiology of SRCS, the clinical presentations of this disease, differential diagnosis with other diseases and pathological conditions accompanied by shoulder pain. A wide range of medications, such as nonsteroidal anti-inflammatory drugs, local glucocorticoid and hyaluronic acid injections, is used to treat SRCS. The review gives the data of clinical trials evaluating the comparative efficiency of these drugs. Special attention is paid to a new method for treating SRCS – the use of platelet-rich plasma (PRP).
