В исследование включены 134 женщины (медиана возраста — 35 [29; 43] лет): 94 пациентки с РЗ [ревматоидным артритом (РА), системной склеродермией (ССД) и псориатическим артритом (ПсА)] и 40 человек без РЗ. Проводилось анкетирование с использованием унифицированной анкеты, денситометрия позвоночника и проксимального отдела бедра с помощью двухэнергетической рентгеновской абсорбциометрии (денситометр Discovery A, Hologic Inc., США), измерение уровня витамина D в сыворотке крови, определение суточного потребления кальция с продуктами питания.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Сниженная МПК была обнаружена достоверно чаще в группе больных РЗ по сравнению со здоровым контролем (25 и 8% соответственно; р=0,0213). При РА она встречалась у 48% больных, при ССД — у 21%, при ПсА — у 15%. У пациенток с РЗ выявлена прямая корреляция МПК во всех областях измерения с ростом, массой тела, индексом массы тела, концентрацией витамина D в сыворотке крови, а обратная — с кумулятивной дозой глюкокортикоидов. Кроме того, выявлена обратная корреляция МПК проксимального отдела бедра с длительностью РЗ. У пациенток с ССД обнаружена обратная зависимость между МПК в обеих областях проксимального отдела бедра и уровнем С-реактивного белка, а у женщин с РА — между МПК в поясничном отделе позвоночника и шейки бедра и СОЭ.</p></sec><sec><title>Заключение</title><p>Заключение. В целом 25% женщин репродуктивного возраста с РЗ нуждаются в наблюдении и профилактике остеопороза, а при наличии переломов — в назначении противоостеопоротического лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to investigate bone mineral density (BMD) status and the factors influencing the latter in reproductive-aged women with rheumatic diseases (RDs).</p></sec><sec><title>Subjects and methods</title><p>Subjects and methods. The investigation enrolled 134 women (mean age, 35 [29; 43] years): 94 patients with RDs (rheumatoid arthritis (RA), systemic sclerosis (SS), and psoriatic arthritis (PsA)) and 40 people without RDs. The investigators conducted a survey using a unified questionnaire, spine and proximal femur bone densitometry by dual energy X-ray absorptiometry (Hologic Discovery A Bone densitometer, USA), serum vitamin D measurement, and daily dietary calcium intake estimation.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Reduced BMD was detected significantly more often in the group of patients with RDs compared with the healthy control (25 and 8%, respectively; p=0.0213). It occurred in 48% of patients with RA, in 21% of those with SS, and in 15% of those with PsA. Patients with RDs showed a direct correlation of BMD in all areas of measurement with height, weight, body mass index, and serum vitamin D concentration and an inverse correlation with the cumulative dose of glucocorticoids. In addition, an inverse correlation was found between proximal femur BMD and RD duration. The patients with SS displayed an inverse relationship between BMD of both proximal femurs and C-reactive protein level; and the women with RA had exhibited this relationship between lumbar spine and femoral neck BMD and ESR.</p></sec><sec><title>Conclusion</title><p>Conclusion.
