Включено 444 пациента (49 женщин, 395 мужчин) в возрасте ≥18 лет c подагрой, не имеющих СД. Медиана длительности наблюдения – 5,66 [2,69; 7,64] года. Для выявления факторов, ассоциирующихся с риском развития СД2, использована многофакторная логистическая регрессия, включившая: пол; наличие СД2 у родственников; недостаточную физическую активность; несбалансированное питание; возраст ≥45 лет; развитие ≥4 приступов подагрического артрита в год; наличие тофусов; индекс массы тела ≥30 кг/м2 ; наличие артериальной гипертензии (АГ); прием аллопуринола, фебуксостата, глюкокортикоидов, диуретиков, метформина, колхицина; скорость клубочковой фильтрации ˂ 60 мл/мин/1,73 м2 ; сывороточный уровень мочевой кислоты (МК) ≥420 мкмоль/л и ≥480 мкмоль/л.</p></sec><sec><title>Результаты</title><p>Результаты. СД2 развился у 108 (24,3%) пациентов. По данным многофакторной модели, увеличивали риск СД2: развитие ≥4 приступов артрита в год (отношение шансов (ОШ) – 5,23; 95%-й доверительный интервал (95% ДИ): 2,98–9,19; р=0,0001); наличие тофусов (ОШ=2,61; 95% ДИ: 1,50–4,54; р=0,001); сывороточный уровень МК ≥480 мкмоль/л (ОШ=2,261; 95% ДИ: 1,02–5,00; р=0,144); прием диуретиков (ОШ=2,35; 95% ДИ: 1,19–4,6; р=0,014). Снижал риск развития СД2 прием фебуксостата (ОШ=0,31; 95% ДИ: 0,11–0,84; р=0,022) и метформина (ОШ=0,49; 95% ДИ: 0,21–1,16; р=0,107).</p></sec><sec><title>Вывод</title><p>Вывод. Риск СД2 у пациентов с подагрой ассоциируется с высокой частотой приступов артрита, уровнем МК ≥480 мкмоль/л, наличием АГ, приемом диуретиков, тогда как применение фебуксостата и метформина снижает риск. </p></sec></abstract><trans-abstract xml:lang="en"><p>It is assumed that the risk of developing type 2 diabetes mellitus (T2DM) in patients with gout is influenced by both generally accepted risk factors and factors related to gout.</p><sec><title>Objective</title><p>Objective. To evaluate the impact of various risk factors for T2DM in patients with gout.</p></sec><sec><title>Material and methods</title><p>Material and methods. 444 patients (49 women, 395 men) ≥18 years old with gout and without DM were included. Duration of observation was 5.66 [2.69; 7.64] g. To identify factors associated with the risk of developing T2DM, multivariate logistic regression was used, which included: sex; T2DM in relatives; insufficient physical activity; unbalanced diet; age ≥45 years; ≥4 attacks per year; presence of tophi; BMI≥30 kg/m2 ; allopurinol, febuxostat, glucocorticoids, diuretics, metformin, colchicine; GFR&lt;60 ml/min/1.73 m2 ; serum uric acid level (sUA) ≥420 μmol/l and ≥480 μmol/l. Results. T2DM developed in 108 (24.3%) patients. Risk of T2DM in patients with gout is associated with high incidence of arthritis attacks, MK≥480 μmol/l, hypertension, diuretic use, and febuxostat and metformin reduces risk.
