Исследование представляет собой субанализ ранее опубликованного систематического обзора и сетевого метаанализа, посвященного сравнительной оценке эффективности генно-инженерных биологических препаратов (ГИБП) у взрослых пациентов с АС в РФ. Рассчитывалось число, которое необходимо пролечить (NNT, number needed to treat) для получения ответа на лечение BASDAI 50 и ASAS 20/40 за 16 недель применения СЕК, ИКСЕ, НТК. Определялся показатель стоимости ответа (CpR, cost per response) для каждого ГИБП за 16 недель и 1 год терапии. Дополнительно была проведена оценка финансового бремени наиболее экономически эффективных стратегий лечения АС на моделируемой популяции.Результаты. Применение ИКСЕ, СЕК и НТК характеризуется необходимостью пролечить в среднем не более 3–5 пациентов для получения одного ответа на терапию ASAS 20/40, BASDAI 50 в зависимости от оцениваемого исхода. По результатам анализа CpR НТК является наиболее экономически эффективным ингибитором ИЛ-17 для лечения АС как в течение 16 недель, так и на протяжении 1 года.Заключение. Полученные результаты позволяют сравнить эффективность ингибиторов ИЛ-17 с клинической и экономической точки зрения и могут быть использованы как при определении тактики лечения отдельных пациентов, так и на популяционном уровне – для обоснования решения о включении препарата в систему возмещения и последующей закупке.</p></abstract><trans-abstract xml:lang="en"><p>Objective – to compare the clinical efficacy and cost-effectiveness of IL-17 inhibitors (SEC, IXE, NTK) in the treatment of adult patients with ankylosing spondylitis (AS) in the healthcare system of the Russian Federation.Material and methods. The study is a sub-analysis of a previously published systematic review and network meta-analysis of the comparative efficacy of biologics in adult patients with AS in the Russian Federation. NNT values were calculated for BASDAI 50 and ASAS 20/40 after 16 weeks of therapy for all studied drugs. CpR was estimated for each biologic after 16 weeks and one year of therapy. Additionally, we carried out an assessment of the financial burden of the most cost-effective strategies for the treatment of AS.Results. The use of NTK is characterized by an average of no more than three patients needed to treat to achieve one ASAS 20/40 or BASDAI 50 response, while on IXE and SEC – no more than 4–5 patients need to be treated, depending on the estimated effectiveness criterion. According to CpR estimate, NTK is the most cost-effective IL-17 inhibitor for the treatment of AS, both after 16 weeks and after one year of therapy.Conclusion.
