Ретроспективное исследование включало 48 пациентов с HCV-КГВ, которые получали терапию ПППД. Активность КГВ в динамике оценивали при помощи Бирмингемской шкалы активности васкулита версии 3 (BVAS, Birmingham Vasculitis Activity Score v. 3). Определялось число больных, у которых достигнут УВО (неопределяемый уровень РНК HCV через 12 недель после окончания терапии ПППД (УВО12)), полный иммунологический ответ (исчезновение криоглобулинов, нормализация уровня ревматоидного фактора и С4-компонента комплемента); полный (0 баллов по шкале BVAS v. 3) или частичный (BVAS v. 3&lt;50% от исходного значения) клинический ответ (КО). Иммуносупрессивная терапия проводилась при необходимости как до, так и после курса лечения ПППД.</p></sec><sec><title>Результаты</title><p>Результаты. Медиана времени наблюдения от момента окончания терапии составила 26,5 [11,5; 62,3] месяца. Все 48 (100%) пациентов достигли УВО. Элиминация криоглобулинов отмечена у 20 (41,7%), полный иммунологический ответ – у 4 (8,3%) больных. Полный КО был достигнут у 13 (27,1%), частичный – у 19 (39,6% ) пациентов. Было показано, что исходная сумма баллов по шкале BVAS v. 3 &lt;4 независимо ассоциирована с полным КО (отношение шансов – 7,58; 95%-й доверительный интервал: 1,42–40,48; р=0,018). Рецидив КГВ был отмечен у 3 (6,3%) пациентов.</p></sec><sec><title>Заключение</title><p>Заключение. Пациенты с HCV-КГВ требуют неопределенно долгого периода наблюдения даже после достижения УВО12. Использование шкалы BVAS v. 3 до начала лечения ПППД может облегчить планирование терапевтической тактики, в частности при определении пациентов, у которых в первую очередь стоит рассмотреть необходимость добавления иммуносупрессивной терапии после эрадикации HCV.</p></sec></abstract><trans-abstract xml:lang="en"><p>Objective – to evaluate the long-term outcomes of HCV eradication with direct-acting antivirals (DAAs) in patients with hepatitis C-associated cryoglobulinemic vasculitis (HCV-CV)</p><sec><title>Materials and methods</title><p>Materials and methods. We retrospectively assessed 48 patients with HCV-CV treated with DAAs. The activity of HCV-CV was assessed by using Birmingham Vasculitis Activity Score version 3 (BVAS v. 3). In patients with HCV-CV the rate of sustained virologic (defined as undetectable HCV-RNA levels 12 weeks after treatment cessation) and immunological (defined as absence of circulating cryoglobulins, rheumatoid factor and normal C4 level) response; and the rate of complete (defined by a BVAS v. 3 score of 0) and partial (defined as BVAS v. 3 score &lt;50% of the baseline score) clinical response were evaluated. Immunosupressants were given before or after DAAs therapy if clinically needed.</p></sec><sec><title>Results</title><p>Results. Median time of follow-up from treatment cessation were 26,5 (11,5–62,3) months. All 48 (100%) patients achieved sustained virologic response. Elimination of cryoglobulins were reported in 20 (41,7%) patients, complete immunological response-in 4 (8,3%) cases. Complete and partial clinical responses were observed in 13 (27,1%) and 19 (39,6%) patients, respectively. BVAS v. 3 score &lt;4 at baseline was independently associated with complete clinical response (ОR=7,58; 95% CI: 1,42–40,48; р=0,018). 3 (6,3%) patients demonstrated HCV-CV relapse.</p></sec><sec><title>Conclusion</title><p>Conclusion. Patients with HCV-CV require a long-term follow-up period even after reaching the SVR.
