В статье приводятся обновленные рекомендации, разработанные Американской коллегией ревматологов и Американской ассоциацией травматологов-ортопедов, по периоперационному лечению больных ревматоидным артритом, анкилозирующим спондилитом, псориатическим артритом, ювенильным идиопатическим артритом и системной красной волчанкой, которым проводится плановое эндопротезирование тазобедренного или коленного суставов. Рассматривается периоперационное применение противоревматической лекарственной терапии, включая базисные противовоспалительные препараты, генно-инженерные биологические препараты, ингибиторы янус-киназ и глюкокортикоиды. Все рекомендации являются условными и основаны на результатах ретроспективных клинических исследований, которые следует учитывать в принятии решений при выборе периоперационной антиревматической терапии. 

The article presents updated guidelines developed by the American College of Rheumatology and the American Association of Hip and Knee Surgeons on the perioperative treatment of patients with rheumatoid arthritis, ankylosing spondylitis, psoriatic arthritis, juvenile idiopathic arthritis and systemic lupus erythematosus undergoing elective total hip or total knee arthroplasty. The perioperative use of anti-rheumatic drug therapy, including traditional disease-modi fying antirheumatic drugs, biologic agents, targeted synthetic small-molecule drugs and glucocorticoids. All recommendations are conditional and based on the results of retrospective clinical studies, which should be taken into account in decisionmaking when choosing perioperative antirheumatic therapy.
