Это связано с широким спектром и большой частотой (до 100%) развития серьезных нежелательных явлений при лечении противоревматическими препаратами этого класса. Однако в реальной практике ГК при РА используются очень часто (в России, странах Западной Европы и США их получают около 50% больных), а лечебные курсы нередко весьма продолжительны. Это объясняется тем, что комбинация базисных противоревматических препаратов (БПВП), включая генно-инженерные биологические препараты (ГИБП) и ингибиторы Янус-киназ (иJAK), с ГК позволяет добиться более быстрого улучшения состояния пациентов и поэтому весьма «удобна» для многих пациентов и лечащих врачей.</p><p>Однако впоследствии обеспечить снижение дозы и (особенно!) полное прекращение приема ГК очень непросто. Клинические и наблюдательные исследования показывают, что отказаться от использования ГК на фоне лечения ГИБП и иJAK удается в среднем у 30–40% пациентов. Кроме того, отмена ГК может существенно повышать риск обострения РА, даже при проведении комплексной патогенетической терапии. Тем не менее, необходимо рассматривать длительный прием ГК, в т. ч. низких доз (менее 7,5 мг/сут. в пересчете на преднизолон) как показатель тяжести течения болезни и неадекватности проводимой базисной терапии. Поэтому следует стремиться к персонификации терапии РА, выбору БПВП (включая ГИБП и иJAK) на основе оценки фенотипа заболевания и предикторов ответа на лечение различными препаратами, добиваясь основной цели терапии – ремиссии/низкой воспалительной активности и достижения приемлемого качества жизни, без использования ГК.</p></abstract><trans-abstract xml:lang="en"><p>Current recommendations for RA treatment determine the need to reduce the dose and duration of glucocorticoid</p><p>(GC) use. This is due to the wide spectrum and high frequency (up to 100%) of serious adverse events (AE) during GC treatment. However, in real practice GCs in RA are used very often (in Russia, Western Europe and the USA they are used in about 50% of patients), and often in long-term courses. This is explained by the fact that the combination of disease-modifying antirheumatic drugs (DMARDs), including biologics (bDMARDs) and Janus kinase inhibitors (JAK), with GCs allows to achieve faster improvement of patients’ condition and therefore is very “convenient” for many patients and physicians.</p><p>However, it is very difficult to ensure dose reduction and (especially!) complete discontinuation of GC administration afterwards. Clinical and observational studies show that 30–40% of patients manage to discontinue the use of GCs during treatment with bDMARDs and JAK. In addition, discontinuation of GCs may significantly increase the risk of RA flare, even with complex pathogenetic therapy.</p><p>Nevertheless, prolonged use of GCs, including low doses (less than 7.5 mg/day of prednisolone) should be considered as an indicator of the severity of the disease course and inadequacy of the current therapy.
