Подавляя активность РА, ОКЗ значительно снижает выраженность основных симптомов РА, которые относятся к параметрам, оцениваемым самим пациентом (ПОСП).</p><p>Цель исследования – оценить влияние олокизумаба на параметры, оцениваемые самим пациентом, по данным метаанализа рандомизированных контролируемых исследований (РКИ) и наблюдательных исследований.</p><sec><title>Материалы и методы</title><p>Материалы и методы. Мы провели поиск публикаций за период с января 2014 г. по июнь 2025 г. в базах данных PubMed, Web of Science, Scopus, Cochrane, eLibrary по ключевым словам «олокизумаб», «ревматоидный артрит», «patients reported outcomes»: боль, PtGA, HAQ-DI, FACIT-F. В дальнейшем был проведен метаанализ серии исследований, соответствующих критериям отбора, в которых, помимо других показателей эффективности ОКЗ, изучалось его влияние на ПОСП: интенсивность боли (по визуальной аналоговой шкале и числовой рейтинговой шкале), оценку активности болезни пациентом (ОАБП), нарушение функции по индексу HAQ-DI (Health Assessment Questionnaire – Disability Index) и утомляемость (по FACIT-F (Functional Assessment of Chronic Illness Therapy – Fatigue)). Оценивалась эффективность ОКЗ в наиболее часто используемом режиме – подкожные инъекции 64 мг каждые 4 нед.</p></sec><sec><title>Результаты</title><p>Результаты. Для проведения метаанализа было отобрано 5 РКИ II–III фазы и 4 наблюдательных исследования в реальной клинической практике (n=3013). Согласно полученным данным, через 12 и 24 нед. от начала терапии на фоне лечения ОКЗ отмечалась выраженная положительная динамика ПОСП. Так, динамика (средняя разность) боли к 24-й нед. составила –33,29 (95% доверительный интервал (95% ДИ): –39,53; –27,07); ОАБП к 24-й нед. –28,62 (95% ДИ: –33,87; –23,37), индекса HAQ к 12-й нед. –0,24 (95% ДИ: –0,35; –0,13), FACIT-F к 24-й нед. 8,80 (95% ДИ: 5,81; 11,79). Полученные данные имели умеренную или высокую гетерогенность (показатель I2&gt;25%). Динамика ПОСП в РКИ и наблюдательных исследованиях носила однонаправленный и близкий характер.</p></sec><sec><title>Заключение</title><p>Заключение. ОКЗ оказывает статистически значимое позитивное влияние на ПОСП при РА. ОКЗ может рассматриваться как эффективное средство для лечения больных РА с хронической болью и утомляемостью.</p></sec></abstract><trans-abstract xml:lang="en"><p>Olokizumab (OKZ) is a monoclonal antibody that binds interleukin 6 in the blood plasma, thereby exerting a pronounced anti-inflammatory effect in rheumatoid arthritis (RA). In addition to its effect on RA disease activity, OKZ significantly reduces the severity of key RA symptoms, which are referred to as patient-reported outcomes (PROs).</p><p>The aim – to evaluate the effect of olokizumab on patient-reported outcomes using data from a meta-analysis of randomized controlled trials (RCTs) and observational studies.</p><sec><title>Materials and methods</title><p>Materials and methods: We searched PubMed, Web of Science, Scopus, Cochrane, and eLibrary for publications from January 2014 to June 2025 using the keywords “olokizumab”, “rheumatoid arthritis”, and “patient-reported outcomes”: pain, PtGA, HAQ-DI, and FACIT-F. A meta-analysis was then conducted of a series of studies that met the selection criteria. In addition to the effect of OKZ on RA disease activity, it also examined its effect on PROs: pain (visual analog scale, 0–100 numeric rating scale), patient global assessment (PtGA), functional impairment (HAQ-DI (Health Assessment Questionnaire – Disability Index)), and fatigue (FACIT-F (Functional Assessment of Chronic Illness Therapy – Fatigue)). The efficacy of OKZ in the most commonly used regimen – subcutaneous injections of 64 mg every 4 weeks – was assessed.</p></sec><sec><title>Results</title><p>Results. Five phase II–III RCTs and four observational studies in real-world clinical practice (n=3013) were selected for the meta-analysis. According to the data obtained, significant positive dynamics in PROs were observed after 12 and 24 weeks from the start of therapy with OKZ treatment. Thus, pain dynamics by 24 weeks (mean difference) was –33.29 (95% confidential interval (95% CI): –39.53; –27.07), PtGA dynamics at 24 weeks was –28.62 (95% CI: –33.87; –23.37), HAQ-DI dynamics at 12 weeks was –0.24 (95% CI: –0.35; –0.13), and FACIT-F at 24 weeks was 8.80 (95% CI: 5.81; 11.79). The data obtained had moderate to high heterogeneity (I2 index &gt;25%). The dynamics of PROs in RCTs and observational studies were unidirectional and similar.</p></sec><sec><title>Conclusion</title><p>Conclusion. OKZ has a significant positive effect on PROs in RA.
