В исследование включено 255 пациентов в возрасте 40–75 лет с достоверным диагнозом ОА КС I–III рентгенологической стадии по Kellgren – Lawrence, подписавших информированное согласие. Средний возраст пациентов составил 53,0±10,5 года, индекс массы тела (ИМТ) – 28,8±6,7 кг/м2 , медиана длительности заболевания – 2 [0,8; 7] года. Проведены анкетирование, лабораторное обследование, выполнены стандартная рентгенография, ультразвуковое исследование КС, магнитно-резонансная томография (МРТ) таргетного сустава, двухэнергетическая рентгеновская абсорбциометрия осевого скелета.</p></sec><sec><title>Результаты</title><p>Результаты. ГУ выявлена в 16,9% случаев. У этих пациентов отмечались более высокие показатели (p&lt;0,05 для всех значений) боли по ВАШ (визуальной аналоговой шкале), суммарного WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index), включая все его составляющие, и ОСЗП (общей оценки состояния здоровья пациента), худшие показатели по шкале KOOS (Knee injury and Osteoarthritis Outcome Score) по сравнению с участниками без ГУ. Кроме того, при наличии повышенного уровня мочевой кислоты статистически значимо чаще регистрировались синовит, нейропатический компонент боли, продвинутые рентгенологические стадии и более выраженные структурные повреждения суставов по данным МРТ (шкала WORMS (Whole-Organ Magnetic Resonance Imaging score)). Однако после коррекции данных по ИМТ и объему талии (ОТ) выявленные различия утратили свою значимость. Вместе с тем, в группе пациентов с ГУ отмечены более высокие показатели минеральной плотности костной ткани проксимального отдела бедра в целом и чаще выявлялась ишемическая болезнь сердца. Также определялась связь с более высокими уровнями триглицеридов, креатинина, щелочной фосфатазы и более низким содержанием липопротеидов высокой плотности (p&lt;0,05 для всех значений). Взаимосвязи с про- и противовоспалительными цитокинами (интерлейкином (ИЛ) 1β, ИЛ-6, ИЛ-10, ИЛ-34), матриксной металлопротеиназой (ММП) 3, ММП-9, маркерами деградации хрящевой и костной ткани (СТХ-I, СТХ-II, СOMP), С-реактивным белком (СРБ)) выявлено не было.</p></sec><sec><title>Заключение</title><p>Заключение. Взаимосвязь между ГУ и тяжестью ОА, наиболее вероятно, носит опосредованный характер и детерминирована избыточной массой тела и висцеральным ожирением (по данным измерения ОТ). Отсутствие связей ГУ с уровнями СРБ, провоспалительных цитокинов и маркеров деградации хряща свидетельствует об отсутствии прямого влияния мочевой кислоты на эти показатели у пациентов с ОА КС.</p></sec></abstract><trans-abstract xml:lang="en"><p>The aim of this single‑center study was to investigate the relationship between hyperuricemia (HU) and biochemical and immunological parameters in patients with knee osteoarthritis (KOA).</p><sec><title>Materials and methods</title><p>Materials and methods. The study included 255 patients aged 40–75 years with a confirmed diagnosis of KOA, Kellgren – Lawrence radiological stages I–III, who provided informed consent. The mean age of the patients was 53.0±10.5 years, body mass index (BMI) was 28.8±6.7 kg/m2 , and the median disease duration was 2 [0.8; 7] years. The following assessments were performed: questionnaire survey, laboratory testing, standard radiography, knee ultrasound, magnetic resonance imaging (MRI) of the target joint, and dual-energy X‑ray absorptiometry of the axial skeleton.</p></sec><sec><title>Results</title><p>Results. HU was detected in 16.9 % of cases. These patients exhibited significantly higher values (p &lt; 0.05 in all cases) for: pain on the visual analogue scale, total WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index) score, including all its components, and general health status, as well as worse scores on the KOOS (Knee injury and Osteoarthritis Outcome Score), compared with participants without HU. Furthermore, elevated uric acid levels were significantly associated with higher incidence of synovitis, neuropathic pain component, advanced radiological stages, and more severe joint structural damage on MRI (WORMS (WholeOrgan Magnetic Resonance Imaging Score)). However, after adjusting for BMI and waist circumference (WC), these differences lost statistical significance. At the same time, patients with HU showed higher bone mineral density values in the proximal femur and a higher prevalence of coronary artery disease. Additionally, associations were found with higher levels of triglycerides, creatinine, alkaline phosphatase, and lower levels of high-density lipoprotein (p &lt; 0.05 in all cases). No associations were found with pro- and anti-inflammatory cytokines (interleukin (IL) 1β, IL-6, IL-10, IL-34), metalloproteinase (MMP) 3, MMP-9, markers of cartilage and bone tissue degradation (CTX-I, CTX-II, COMP), or C‑reactive protein.</p></sec><sec><title>Conclusion</title><p>Conclusion. The relationship between HU and KOA severity is most likely indirect and is determined by excess body weight and visceral obesity (as indicated by WC).
