На основании иммунно-морфологических данных определить участие тубулоинтестцильного и гломерулярного компонентов в прогрессировании почечной патологии у больных системной красной волчанкой (СКВ). Материал и методы. Проводили исследование у 45 пациентов СКВ с люпуснефритом, 34 больных хроническим гломерулонефритом (ХГН), 19 больных лекарственным хроническим тубулоинтерстициальным нефритом (ХТИН) и 30 лиц контрольной группы. Иммуноферментным методом определяли в сыворотке крови концентрации: антител к нативной ДНК (а-т к ДНК), С-реактивного белка (СРБ), трансформирующего фактора роста бета-1 (ТФР-β1), фактора некроза опухоли – α (ФНО-α), моноцитарного хемотаксического протеина-1 (МХП-1). Морфологический анализ включал исследование биоптатов почек с полуколичественной оценкой показателей, отражающих активность процесса, склероз, изменения сосудов почек.Результаты. Отмечалось увеличение концентрации показателей ФНО-α и СРБ у больных СКВ, ХГН и ХТИН. Однако наиболее высокие значения этих показателейнаблюдались у больных с ХТИН, самые низкие – у больных ХГН, пациенты с СКВ занимали по этим показателям промежуточное положение. Для ФНО-α получена четкая тенденция к росту его значения в зависимости от степени активности СКВ. Высокие цифры МХП-1 были зафиксированы у больных с люпус-нефритом и ХТИН. TФР-β1 у больных с СКВ был достоверно снижен. При балльной оценке морфологических изменений нефробиоптата отмечено, что гломерулярный компонент люпус-нефрита характеризовался в основном умеренным активным пролиферативным и умеренным склеротическим процессами, а тубулоинтерстициальный компонент – преобладанием выраженных изменений. Выводы. Поражение почек при системной красной волчанке характеризуется сочетанными изменениями клубочков и интерстициальной ткани. При этом отмечается значительно выраженный тубулоинтерстициальный компонент. Важную роль в формировании патологического процесса играют МХП-1 и ТФР-β1, изменения которых связаны с активностью волчаночного процесса.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To assess participation of tubulointerstitial and glomerular components in renal damage progression in pts with systemic lupus erythematosus (SLE) using immuno-morphological data. Material and methods. 45 SLE pts with lupus nephritis, 34 pts with chronic glomerulonephritis (CGN), 19 pts with drug induced chronic tubulointerstitial nephritis (DCTN) and 30 control persons were included. Concentration of anti-DNA antibodies, C-reactive protein (CRP), transforming growth factor β (TGFβ), tumor necrosis factor α (TNFα), monocytic chemotactic protein 1 (MCP1) was evaluated by immunoenzyme assay. Morphological examination of renal biopsies with semiqantitative assessment of measures reflecting process activity, sclerosis and renal blood vessels changes was performed. Results. Elevated concentration of TNFα and CRP was revealed in pts with SLE, CGN and DCTN. Pts with DCTN had highest, pts with CGN – lowest and SLE pts – intermediate values of these measures. There was a clear tendency of TNFα elevation with increase of SLE activity. Pts with lupus nephritis and DCTN had high values of MCP1. TGFβ level was significantly decreased in SLE. Glomerular component of lupus nephritis was characterized mainly by moderate proliferation and sclerosis while tubulointerstitial component signs were prominent. Conclusion.
