Изучить роль воспаления в развитии автономной кардиальной дисфункции, оцененной методом суточной вариабельности ритма сердца (ВРС), у больных системной красной волчанкой (СКВ). Материал и методы. Обследовано 75 пациенток с СКВ в возрасте до 55 лет. Контрольная группа — 32 здоровых женщин, сопоставимых с больными по возрасту. Помимо клинических проявлений и степени активности СКВ, оценивалось наличие основных кардиоваскулярных факторов риска, проводились дуплексное сканирование общих сонных артерий с измерением толщины комплекса «интима- медиа», эхокардиграфия, суточное мониторирование ЭКГ с расчетом временных показателей ВРС, определение уровня сывороточных маркеров воспаления (СРВ, ИЛ-6, ФНО-а). Результаты. Временные параметры ВРС, нормированные по средней длительности нормальных интервалов NN (SDNNi%, RMSSD%, pNN50%) у женщин с СКВ были ниже, чем у здоровых лиц. Выявлена обратная корреляция параметров ВРС с маркерами воспаления при СКВ (р&lt;0,05). Пациентки с СКВ были разделены на 4 группы: 1 (п=12) — со сниженной ВРС и высокими значениями ИЛ-6; 2 (п=13) —со сниженной ВРС и нормальным уровнем ИЛ-6; 3 (п=6) — с нормальной ВРС и повышенной концентрацией ИЛ-6; 4 (п=44) — с нормальными значениями ВРС и ИЛ-6. У больных СКВ группы 1 выявлены меньшая длительность заболевания, более высокие значения индекса SLEDAI 2К, размеров левого предсердия, маркеров воспаления по сравнению с пациентками группы 4 (р&lt;0,05). У больных группы 2 отмечена более высокая частота артериальной гипертензии, дислипидемии, курения, менопаузы и атеросклеротических изменений в сонных артериях по сравнению с группой 4 (р&lt;0,05). Заключение: Аутоиммунное воспаление играет важную роль в развитии автономной кардиальной дисфункции при обострении СКВ, а традиционные факторы риска и атеросклеротическое поражение сосудов — при невысокой степени активности заболевания. Совместное определение временных параметров ВРС и маркеров воспаления при СКВ с учетом тяжести заболевания и оценкой традиционных факторов риска может помочь в прогнозировании риска сердечно-сосудистых осложнений у больных СКВ.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To evaluate role of inflammation in pathogenesis of cardiac autonomic dysfunction assessed by heart rate variability (HRV) analyses, in patients (pts) with systemic lupus erythematosus (SLE). Material and methods. 75 female pts with SLE aged before 55 years were included. Control group consisted of 32 healthy women of comparable age. Clinical symptoms, activity degree and main cardiovascular risk factors were assessed. Common carotid duplex scanning with measurement of intima-media thickness, echocardiography, 24 h ECG ambulatory recording with determination of time domain parameters, evaluation of serum inflammation markers level (CRP, IL-6, TNFa) were performed. Results. HRV time domain parameters adjusted by McanNN - SDNNn%, SDNNin%, rMSSDn%, pNN50% in women with SLE were lower than in healthy control. HRV parameters inversely correlated with inflammation markers in SLE (p&lt;0,05). SLE pts were divided into 4 groups: 1 (n=12) - pts with low HRV parameters and high level of IL-6; 2 (n=13) - low HRV and normal IL-6; 3 (n=6) - normal HRV and high IL-6; 4 (n=44) - normal HRV and IL-6. SLEDA1 2K, left atrium size, levels of markers of inflammation in group 1 were higher, then in group 4 (p&lt;0,05). Hypertension, dyslipidemia, smoking and subclinical atherosclerosis of carotid arteries were more frequent in group 2 compared to group 4 (p&lt;0,05). Conclusion. These results suggest important role of autoimmune inflammation in pathogenesis of cardiac autonomic dysfunction in SLE pts with high disease activity and traditional cardiovascular risk factor, atherosclerotic changes - in low SLE activity.
