ПРИМЕНЕНИЕ РИТУКСИМАБАУ БОЛЬНЫХ ВОЛЧАНОЧНЫМ НЕФРИТОМ

Аннотация

Цель. Оценить влияние терапии ритуксимабом (РТМ) на клинические, морфологические и иммунологические показатели ак-
тивности волчаночного нефрита (ВН).
Материал и методы. В исследование включено 19 больных ВН (III и IV тип ВН по ВОЗ) с длительностью болезни 6-133 мес.
Средний возраст составил 28,6 года (18-63 года). РTМ назначался в дозе 500 мг еженедельно в течение 4 нед или дважды по 1000
мг с интервалом 2 нед. Полный курс терапии РTМ получили 14 больных. В 5 случаях курс терапии не был завершен в связи с плохой
переносимостью или гибелью больного. Повторные биопсии почки были проведены у 8 больных через 6-12 мес от начала лечения.
Результаты. Частичная ремиссия ВН на разных сроках наблюдения была получена у 13 больных. Через год после курса РTМ
полная ремиссия отмечалась у 7 больных. Случаев обострения ВН у больных, достигших ремиссии, не зафиксировано. Снижение
активности по SLEDAI 2K наблюдалось уже через 1 мес после терапии - с 20,47,3 до 14,36,5 (р<0,05). Достоверное сниже-
ние титров антител к дсДНК и нормализация С3 и С4 определялось к 3-6-12-му месяцам. В целом терапия РTМ оказалась
эффективной у 13 из 19 больных (68%). При повторном морфологическом исследовании биоптата почки во всех случаях выявле-
но значительное уменьшение индекса морфологической активности в среднем с 8 (6-9) до 3 (1-4), р=0,027. Трансформация
типа ВН в более благоприятный выявлена в 5 случаях. Терапия РTМ оказалась неэффективной у 6 больных с выраженной по-
чечной недостаточностью. Серьезных побочных реакций и осложнений после применения РTМ не наблюдалось.
Выводы. Применение терапии РTМ в большинстве случаев приводит к снижению клинической, лабораторной и морфологиче-
ской активности ВН при условии сохранности азотовыделительной функции почек на момент начала терапии.
