Современные стандарты лабораторной диагностики ревматических заболеваний и их применение в реальной клинической практике

Проанализировано применение современных стандартов лабораторной диагностики ревматических заболеваний (РЗ) в клинической практике по данным лаборатории иммунологии и молекулярной биологии ревматических заболеваний ФГБУ «НИИР им. В.А. Насоновой» РАМН за 2012 г. Всего обследовано 19 900 больных РЗ, произведено 90 364 анализа (в среднем по 4,5 теста на 1 пациента); 63,6% (57 338) анализов составило определение аутоантител; 12,9% (11 661) - белков острой фазы воспаления; 25,5% (21 365) -других биомаркеров. На антинуклеарные антитела выполнено 27 026 (30%) анализов, на ревматоидный фактор - 7027 (7,8%), на антитела к циклическому цитруллинированному пептиду - 8194 (9,1%), на антифосфолипидные антитела - 8353 (9,2%), на антинейтрофильные цитоплазматические антитела - 2550 (2,8%). Измерение уровня С-реактивного белка составило 12,0% (10 845) анализов; IgG, IgG4, IgM, IgA -7,9% (7178); комплемента - 5,5% (4978); криоглобулинов - 2,9% (2578); субпопуляций лимфоцитов - 1,1% (950); антистрептолизина-О - 1,3% (1156). Показано, что среди лабораторных биомаркеров РЗ наибольшее клиническое значение имеют аутоантитела и острофазовые показатели. В реальной клинической практике диагностическая чувствительность и специфичность, отношение правдоподобия положительных и отрицательных результатов лабораторных тестов могут отличаться от таковых при изучении специально отобранных групп пациентов и здоровых лиц. В связи с этим назначение и оценка результатов лабораторных исследований должны проводиться в строгом соответствии с предполагаемым диагнозом и данными тщательного клинического обследования больных.
