Остеоартроз суставов кистей: к вопросу о качестве жизни и эстетическом дискомфорте

Аннотация
Остеоартроз (ОА) – одна из наиболее частых причин болей в кистях, приводящая к снижению качества жизни (КЖ). Помимо боли и нарушения функции важное значение имеет развитие у пациентов эстетической неудовлетворенности, которая в настоящее время никак не определяется и не измеряется.
Цель – оценить ряд аспектов КЖ и измерить уровень эстетического дискомфорта у пациентов с ОА суставов кистей.
Материал и методы. Включено 60 женщин 45–75 лет с ОА суставов кистей. Определялось число болезненных и деформированных суставов, оценивались функциональные нарушения с помощью опросника AUSCAN. Больные заполняли анкету для определения уровня их неудовлетворенности внешним видом кистей; выполнялся стресс-тест Эллиса, выявляющий наличие у пациенток иррациональных установок.
Результаты. У пациенток с ОА кистей выявлен значительный уровень эстетического дискомфорта, сопоставимый с уровнем боли в суставах. Наиболее выраженными были боязнь деформации в будущем и внешнее неудобство из-за деформации кистей; наименее выраженным – желание хирургической коррекции. При сравнении результатов, полученных в разных возрастных группах, самые высокие показатели беспокойства и желания хирургической коррекции определялись у более молодых женщин. Также на эстетическую неудовлетворенность пациенток влияли наличие работы и пользование интернетом.
Заключение. Эстетический дискомфорт – значимая составляющая сниженного КЖ у пациенток с ОА кистей. Необходимы дальнейшее изучение этого фактора и разработка критериев его оценки.
