Значение артрита периферических суставов в клинической картине анкилозирующего спондилита

Аннотация
Цель – выяснить роль артрита периферических суставов в клинической картине анкилозирующего спонди- лита (АС) и его влияние на терапевтическую тактику в реальной практике врачей ревматологов.
Материал и методы. В исследование включено 330 больных АС, последовательно обратившихся к ревматоло- гам в течение 4 мес в 24 городах Российской Федерации. На всех больных заполнялась специально разрабо- танная клиническая карта.
Результаты. Периферический артрит имели 47% из 300 больных АС, в том числе у 17% имелось поражение сус- тавов верхних, а у 46% – нижних конечностей. У пациентов с периферическим артритом были выше значение СОЭ, индексы BASDAI и ASDAS-СОЭ. У них также отмечались более выраженные функциональные наруше- ния по сравнению с больными, имевшими изолированный аксиальный вариант заболевания. У 56% больных были выявлены клинические признаки поражения тазобедренных суставов, а у 43% они были двусторонними. По мнению ревматологов, 24 (8%) больных нуждались в эндопротезировании тазобедренного сустава. Выводы. Наличие периферического артрита утяжеляет течение АС.
