Принципы хирургического лечения больных с остеоартритом I плюснефалангового сустава

Аннотация

В настоящее время не существует общего подхода к выбору хирургической тактики при hallux rigidus. Используется множество способов хирургического лечения остеоартрита (ОА) I плюснефалангового сустава (I ПФС), актуальных при разных стадиях заболевания. Тем не менее данный факт также свидетельствует о том, что все предложенные методы имеют те или иные недостатки. При этом «золотым стандартом» остается артродез I ПФС, избавляющий пациентов от боли, но в функциональном плане уступающий суставосберегающим операциям. Однако в связи с тем, что ОА I ПФС нередко страдают не только лица старше 50 лет, но и более молодые пациенты, наиболее щадящим вариантом суставосберегающих операций представляется хейлэктомия с хондропластикой I ПФС, позволяющая восстановить безболезненные движения в суставе, при этом не изменяя анатомию стопы. Хондропластика методом аутологичного матрикс-индуцированного хондрогенеза (Authologous Matrix Induced Chondrogenesis – AMIC®) описана при наличии дефектов хряща в коленном, тазобедренном и голеностопном суставах. Описание хондропластики I ПФС методом AMIC не встречается ни в отечественной, ни в зарубежной литературе.
