Долгосрочная эффективность нетакимаба в терапии рентгенологического аксиального спондилоартрита: результаты систематического обзора и симуляционных непрямых сравнений

Аннотация

Цель исследования – сравнение долгосрочной эффективности нетакимаба с таковой у других зарегистрированных в Российской Федерации препаратов для терапии рентгенологического аксиального спондилоартрита на горизонте до 3 лет.

Материал и методы. Проведен систематический поиск литературы в базах PubMed и Embase. Отобрано 19 публикаций, оценивающих результаты 11 клинических исследований. Для сравнения использован метод симуляционного непрямого сравнения, согласно рекомендациям NICE DSU TSD 18 (National Institute for Health and Care Excellence Decision Support Unit Technical Support Document 18). Вмешивающиеся переменные для коррекции: возраст, пол, BASDAI (Bath Ankylosing Spondylitis Disease Activity Index), ASDAS (Ankylosing Spondylitis Disease Activity Score), С-реактивный белок, длительность заболевания после постановки диагноза, статус HLA-B27.

Результаты. Нетакимаб продемонстрировал превосходство над всеми ингибиторами фактора некроза опухоли α и интерлейкина 17, а также над тофацитинибом по основным конечным точкам эффективности ASAS20 и ASAS40 (20%и 40%-е улучшение соответственно по критериям Assessment of SpondyloArthritis international Society) на горизонте до 3 лет.

Заключение. Нетакимаб является одним из наиболее эффективных среди доступных генно-инженерных биологических препаратов (ГИБП) для длительного лечения активного аксиального спондилоартрита. Использование нетакимаба в качестве стартового ГИБП позволяет с наибольшей вероятностью достичь целей лечения. Результаты полезны для принятия обоснованных клинических решений при определении тактики применения ГИБП с учетом профиля пациента.
