Частота определения и диагностическое значение антител к циклическому цитруллинированному пептиду (АЦЦП) и антител к модифицированному цитруллинированному виментину (АМЦВ) у детей с ранним ювенильным артритом

Резюме D Цель: Определить частоту выявления повышенных уровней антител к циклическому цитруллинированному пептиду (АЦЦП) и антител к модифицированному цитруллинированному виментину (АМЦВ) и их диагностическую значимость у детей с ранним ювенильным артритом (ЮА). Материал и методы. АЦЦП были определены в сыворотках крови 80 пациентов с ранним (ЮА) (36-девочек, 44-мальчика, ср. возраст-8,5+5,03 лет), АМЦВ — у 85 больных с ранним ЮА (49-девочек, 36-мальчиков) в возрасте от 1,5 до 16 лет (ср. 8,7 ± 4,9 лет). Длительность заболевания у всех детей — менее 6 мес. Группа сравнения — 54 взрослых пациентов с ранним РА (рРА) и 27 — с недифференцированным артритом (НДА), 37 условно здоровых детей. Концентрацию АЦЦП определяли ИФМ с помощью коммерческих наборов “ Axis Shield Diagnostics” (Великобритания), верхняя граница нормы — 5,0 ЕД/мл. Концентрацию АМЦВ — ИФМ с помощью коммерческих тест-систем “Orgentec Diagnostics” (Германия). Верхняя граница нормальной концентрации — 25 ЕД/мл. Результаты. Повышенные уровни АЦЦП были выявлены у 7 детей с ранним ЮА (8,8%). Частота была выше, чем у здоровых детей, однако ниже, чем у взрослых пациентов с рРА и сопоставима с НДА. АЦЦП определялись чаще при ЮРА, чем при ЮХА, у РФ позитивных больных. Концентрация была выше у больных с РФ позитивным и полиартику- лярным ЮА. Повышение уровней АМЦВ было выявлено у 23 (27,1 %) больных с ранним ЮА (чаще, чему здоровых доноров, однако реже, чем у взрослых больных с рРА и НДА). Достоверно чаще у пациентов с ЮРА обнаруживались повышенные уровни АМЦВ, чем при ЮОД, у РФ позитивных, чем у РФ негативных. Концентрация АМЦВ была выше, чем у здоровых, но ниже, чем у взрослых, выше при РФ+, чем при РФ- ЮА. Были обнаружены корреляции АЦЦП, АМЦВ с ЧПС, ЧБС, РФ, а АМЦВ также с СОЭ и СРБ. Выводы. АЦЦП и АМЦВ выявляются у пациентов с ранним ЮА с равной или большей частотой, чем РФ. Несмотря на низкую чувствительность, они обладают высокой специфичностью для диагностики ЮРА в отличие от ЮХА. Определение АЦЦП изолированно или в комбинации с РФ и АМЦВ имеет значение для прогнозирования полиартикулярного поражения, потенциально опасного в плане развития значительных функциональных нарушений, и своевременного назначения адекватной, нередко агрессивной терапии на ранних сроках болезни.
