Стрессовые факторы и депрессивные расстройства при ревматических заболеваниях

Аннотация
В статье обсуждается проблема частой коморбидности иммуновоспалительных ревматических заболеваний (РЗ) и депрессии. Рассматриваются причины и механизмы, объединяющие данные заболевания, а именно: провоцирующая роль хронического психосоциального стресса, нарушения нейроэндокринной регуляции иммунного ответа, приводящие к гиперпродукции провоспалительных цитокинов, определяющих выраженность основных клинических синдромов РЗ и депрессии — хронической боли, усталости, нарушений сна, функциональной недостаточности. Обсуждается влияние депрессии на приверженность пациентов лечению и эффективность проводимой терапии РЗ, а также на течение и исход РЗ. Особое внимание уделяется вопросу своевременной терапии депрессии при РЗ, влиянию генно-инженерных биологических препаратов на депрессивную симптоматику, необходимости персонифицированного подхода к назначению антидепрессантов. Учитывая важность своевременного выявления и лечения депрессивных расстройств в ревматологической практике с клинических позиций и с точки зрения медицинских, социальных и экономических последствий, предлагается междисциплинарный подход к ведению больных РЗ с участием ревматологов, психиатров, неврологов и медицинских психологов.
