Индекс тяжести у больных системной красной волчанкой

Цель исследования – оценка клинического значения и прогностической информативности ИТ у больных СКВ.

Материал и методы. Проанализировано 146 больных мужского пола с достоверным диагнозом СКВ в возрасте от 15 до 64 лет, наблюдавшихся в течение 15-летнего периода. Больным проводилось общепринятое клиническое, лабораторное и инструментальное обследование с использованием стандартных методов оценки активности заболевания. Для характеристики течения и исходов СКВ использовались ИТ и индекс повреждения SLICC/ACR Damage Index (SDI). ИТ и индекс повреждения на 1, 5, 10-й годы заболевания анализировались в динамике у 133, 91 и 63 больных соответственно.

Результаты. Выявлены достоверные корреляции между ИТ на момент включения и показателями, отражающими течение и активность СКВ. Анализ динамики ИТ показал, что в течение первых 5 лет болезни максимальное его нарастание наблюдалось у больных с возрастом дебюта СКВ <20 лет. При сочетании СКВ и вторичного антифосфолипидного синдрома отмечалось более выраженное нарастание ИТ по сравнению с больными, которые не имели таких нарушений. Максимально высокие показатели выживаемости отмечены у больных с «ранним» ИТ, равным 0.

Вывод. ИТ является хорошим показателем, отражающим «накопленную» активность заболевания. Он нарастает с увеличением давности заболевания, при этом его увеличение может происходить на любых стадиях болезни. Эта динамика зависит от варианта течения заболевания, сроков установления диагноза, сопутствующей патологии. ИТ хорошо коррелирует с традиционно используемыми при СКВ показателями активности и повреждения, а также частотой обострений заболевания, что свидетельствует о возможности использования ИТ в качестве прогностического маркера.
