Псориатический артрит: классификация, клиническая картина, диагностика, лечение

Аннотация

Псориатический артрит (ПсА) – хроническое воспалительное заболевание суставов, позвоночника и энтезисов из группы спондилоартритов (СпА), которое обычно наблюдается у больных псориазом (Пс). Диагноз ПсА устанавливается с помощью критериев псориатического артрита (CASPAR). Заболевание является результатом взаимодействий между генетическими, иммунологическими факторами и факторами внешней среды. К основным клиническим проявлениям ПсА относят: периферический артрит, энтезит, дактилит, спондилит. ПсА необходимо дифференцировать с ревматоидным артритом, подагрой, реактивным артритом, остеоартритом, анкилозирующим спондилитом. В связи с тем что ПсА является клинически гетерогенным заболеванием, оценка его активности осуществляется с помощью комплексных индексов, с учетом наличия у больного артрита, энтезита, дактилита, спондилита. Целью лечения ПсА является достижение ремиссии или минимальной активности основных клинических проявлений заболевания, замедление или предупреждение рентгенологического прогрессирования, увеличение продолжительности и качества жизни пациентов, а также снижение риска коморбидных заболеваний, что достигается с помощью широкого спектра препаратов различных классов. Терапию следует выбирать с учетом клинических проявлений ПсА и коморбидных состояний пациентов.
