БЫСТРЫЙ ЭФФЕКТ ТОЦИЛИЗУМАБА ПРИ РЕВМАТОИДНОМ АРТРИТЕ

Цель - оценить эффективность двух инфузий тоцилизумаба (ТЦЗ) у пациентов с ревматоидным артритом (РА). Материал и методы. В исследование включено 43 пациента преимущественно женского пола (м:ж - 10:33), среднего возраста, с длительностью РА 5,6±4,74 года, серопозитивные по ревматоидному фактору (РФ) и антителам к циклическому цитруллинированному пептиду (АЦЦП), с высокой активностью воспалительного процесса до начала терапии ТЦЗ (DAS 28 - 6,3), которые получали различные базисные противовоспалительные препараты, в том числе метотрексат и глюкокортикоиды, без достаточного терапевтического эффекта. Лечение ТЦЗ осуществлялось по стандартной схеме, доза препарата составила 8 мг/кг на каждую инфузию. Эффективность терапии оценивали через 4 нед после каждой инфузии по динамике индекса DAS 28, его отдельных компонентов и HAQ.
Результаты. Установлено, что у пациентов с РА после первой инфузии ТЦЗ наблюдалась достоверная положительная динамика основных клинико-лабораторных параметров активности заболевания (p<0,0001 во всех случаях), которая нарастала после второй инфузии ТЦЗ. Отмечено снижение DAS 28 до 4,1 и 3,1 после каждой инфузии, уменьшение числа пациентов с высокой и увеличение числа пациентов с низкой/умеренной активностью заболевания. При этом после второй инфузии препарата более чем у 20% пациентов развилась ремиссия заболевания. Хороший эффект терапии отмечен после первой инфузии у 14% пациентов, после второй инфузии - у 51%.
Выводы. ТЦЗ оказывает быстрое статистически значимое положительное влияние на основные клинические и лабораторные параметры активности РА.
