Сравнительная оценка влияния ингибиторов фактора некроза опухоли а и нестероидных противовоспалительных препаратов на воспалительные изменения в позвоночнике по данным магнитно-резонансной томографии у больных анкилозирующим спондилитом

Аннотация

Цель - оценить динамику воспалительных изменений (ВИ) позвоночника и крестцово-подвздошных суставов (КПС) по данным магнитно-резонансной томографии (МРТ) у пациентов с анкилозирующим спондилитом (АС) на фоне лечения ингибиторами фактора некроза опухоли а (ФНО α) или нестероидными противовоспалительными препаратами (НПВП), назначенными впервые. Материал и методы. МРТ наиболее болезненного отдела позвоночника и КПС была проведена 58 пациентам с АС исходно и через 12 нед. Использовались режимы МРТ T2 STIR и T1 (SIGNA EXCITE, General Electrics; 0,35 Т, матрица 288×192). Наличие отека костного мозга расценивалось как активное воспаление. Для оценки активных ВИ в наиболее болезненном отделе позвоночника использовался метод счета modified ASspiMRI-a, в КПС - Лидский метод счета. МРТ оценивалась независимо, двумя специалистами, один из них не знал о номере визита и проводимом лечении. Пациенты были разделены на две группы, в первую вошли пациенты, которым впервые назначались ингибиторы ФНО α, в другую - те, которым впервые назначались НПВП. Обе группы были сопоставимы по демографическим показателям. Результаты. Средний уровень боли в исследуемом отделе позвоночника достоверно снизился в обеих группах: с 5,7±1,7 до 2,3±1,8 в группе ингибиторов ФНО α (р=0,000006), с 4,8±2,3 до 2,6±2,3 в группе НПВП (р=0,00001). Значительное уменьшение МРТ-признаков ВИ в позвоночнике через 12 нед лечения отмечалось у пациентов, получавших ингибиторы ФНО а (n=28), с 4,8±2,3 до 1,6±1,6 (p=0,00001); при этом у больных (n=17) с исходно большим числом очагов ВИ (>5) эта динамика была более выражена, чем в группе (n=11) с исходно меньшим количеством ВИ (<5) (средняя величина ∆ соответственно 4,3±1,5 и 1,6±1,4; р=0,0003). У пациентов, получавших НПВП (n=30), уменьшение числа ВИ на МРТ позвоночника было недостоверным (с 2,8±2,5 до 2,3 ±2,1; р=0,17). Регресс активного сакроилиита отмечался через 12 нед у пациентов из группы ингибиторов ФНО α (n=28) со снижением Лидского счета с 2,1±2,0 до 0 (p=0,01). Исходно 28,5% пациентов имели признаки активного сакроилиита на МРТ, через 12 нед они не выявлены ни в одном случае. Уменьшение активного сакроилиита наблюдалось также у пациентов из группы НПВП (n=30), но недостоверно, Лидский счет снизился с 2,6±2,5 до 2,1±2,0 через 12 нед (р=0,083). Исходно признаки активного сакроилиита на МРТ имели 73,3%, через 12 нед - 60% больных. Среднее уменьшение счета ВИ по данным МРТ за время наблюдения на фоне лечения ингибиторами ФНО α было достоверно более выраженным, чем при использовании НПВП (p=0,003 для позвоночника и p=0,013 для КПС). Заключение. Интенсивность боли в исследуемом отделе позвоночника достоверно снижается как при назначении НПВП, так и при использовании ингибиторов ФНО α. ВИ по данным МРТ достоверно уменьшаются при лечении ингибиторами ФНО α, но не НПВП. При большей исходной выраженности ВИ на МРТ их уменьшение на фоне лечения ингибиторами ФНС α было более значительным.
