Факторы риска кардиоваскулярных заболеваний при ревматоидном артрите

Основная причина преждевременной летальности при РА – сердечно сосудистые осложнения (ССО) (ИМ, внезапная смерть, ОНМК). Ключевая роль в развитии ССО при РА принадлежит хроническому воспалению. Цель: Проанализировать распространенность ССЗ у больных РА, значение традиционных ФР и БПВП, клинических проявлений РА в развитии атеросклероза и связанных с ним заболеваний, сравнить результаты с данными QUEST-RA. Материал и методы: Проведен скрининг традиционных ФР и встречаемости ССЗ у пациентов с РА. Пациенты заполняли опросник, включавший оценку традиционных ФР ССЗ. ИБС, ИМ и ОНМК устанавливали на основании данных медицинской документации. Субклинические проявления атеросклероза выявляли с помощью УЗ сканирования сонных артерий. Результаты: Скрининг традиционных ФР ССЗ проведен у 563 пациентов (496 жен. и 67 муж.), в возрасте 47,6±0,37 лет, c длительностью болезни 99,4±3,7 мес.4 Клинические проявления АТ встречались у 5,6%, субклинические – у 11% пациен- тов РА. ГЛП выявлена у 82%, повышение ИМТ – у 51%, отягощенная наследственность по ССЗ – у 44%, АГ – у 38% пациентов РА. Традиционные ФР, внесуставные проявления РА, ССО и ранние признаки АТ чаще наблюдались у мужчин, чем у женщин (р<0,05). У мужчин толщина КИМ была больше, чем у женщин (р<0,05). Для оценки взаимосвязи между традиционными ФР и ССО больные РА разделены на 2 группы: I – с ССЗ, II – без ССЗ. Частота традиционных ФР – АГ, СД и повышенного ИМТ была выше у пациентов I группы, по сравнению со II. Относительный риск их развития составил 4,78, 3,06 и 2,09 соответственно (р<0,05). У 38% больных РА с ССЗ наблюдались системные проявления РА (ОР=2,02; р=0,04). Установлена связь между длительностью приема плаквенила, сульфасалазина и толщиной КИМ (r=0,34; р=0,0002 и r=0,28; р=0,008, соответственно). Ассоциации ССО с применением других БПВП не выявлено. Вывод. При РА наблюдается увеличение риска ССЗ, обусловленных атеросклеротическим поражением сосудов, которые во многом определяются традиционными кардиоваскулярными факторами риска и патогенетическими механизмами разви- тия заболевания.
