Провоспалительные цитокины и депрессия при ревматоидном артрите

Цель исследования — уточнение связи между уровнями провоспалительных цитокинов: фактора некроза опухоли а (ФНОа), интерлейкина 1β (ΚΓ1β), ИЛ6 — и наличием и выраженностью расстройств тревожнодепрессивного спектра (РТДС) у больных ревматоидным артритом (РА). Материал и методы. В исследование включены 45 пациенток с достоверным диагнозом РА. Средний возраст составил 45,5+3,09 года, средняя длительность болезни — 155,0+26,5 мес. Активность РА определяли с помощью индекса DAS28, выраженность усталости — по шкале FSS, выраженность боли — по шкале BPI. Психические расстройства диагностировались психиатром в соответствии с МКБ-10 и DSM-IV c использованием ряда психиатрических и психологических шкал и методик. Концентрацию провоспалительных цитокинов определяли с использованием технологии xMAP (27-plex) на анализаторе BioPlex-200 (Bio-Rad, США). Результаты. Психические РТДС были выявлены у 82,2%, умеренные когнитивные нарушения (КН) — у 67,7% пациентов. 80% обследованных больных отмечали клинически значимую усталость, 64,5% испытывали сильную и умеренную боль. Уровень ФНОа и ИЛ1β был несколько выше у пациентов с РТДС и КН, чем у больных, не имевших таких нарушений. Концентрация ИЛ6 была наиболее высокой при наличии тревожных расстройств. Однако эти различия статистически не значимы. Уровень ФНОа был несколько выше у пациентов с клинически значимой усталостью и значимо выше у больных, испытывающих сильную боль. Уровень ИЛ1 β был достоверно выше у пациентов с сильной и умеренной болью, чем при слабой боли (р<0,05). Заключение. РТДС и КН у больных РА ассоциируются с увеличением уровней провоспалительных цитокинов (ФНОа, ИЛ1β и ИЛ6), что подтверждает роль хронического воспаления в патогенезе РТДС и КН при РА. Высокий уровень ФНОа и ИЛ1β характерен для пациентов с РА с клинически значимой усталостью и выраженной болью.
