Ремиссия при аксиальных спондилоартритах – определение и инструменты оценки (рекомендации Экспертной группы по изучению спондилоартритов при Общероссийской общественной организации «Ассоциация ревматологов России»)

В статье представлено определение ремиссии при аксиальных спондилоартритах (аксСпА), разработанное Экспертной группой по изучению спондилоартритов (ЭкСпА). В работе был использован принцип Дельфийского консенсуса. На первом этапе эксперты на основании данных анализа литературы и собственного клинического опыта предложили варианты определения ремиссии и способы ее оценки у больных аксСпА. На втором этапе путем анонимного голосования были отобраны варианты, получившие не менее 80% голосов, которые были приняты без дополнительного обсуждения. Варианты, получившие равное количество голосов, были пересмотрены, дополнительно обсуждены, после чего проведено повторное голосование с выбором окончательного их варианта. В итоге обсуждения были сформулированы определения клинико-лабораторной и определяемой по результатам магитно-резонансной томографии (МРТ) ремиссии. Предложено выделять следующие варианты ремиссии при аксСпА: лекарственную и безлекарственную, клинико-лабораторную и МРТ-ремиссию, а также ремиссию с наличием и отсутствием структурных изменений опорно-двигательного аппарата по данным визуализирующих методов исследования. Были разработаны критерии клинико-лабораторной ремиссии и основные инструменты ее оценки, которые можно использовать в реальной клинической практике и в научных исследованиях.
