Факторы риска кардиоваскулярных нарушений и атеросклероза при системной красной волчанке

Аннотация
Резюме Цель. Выявить частоту и значимость факторов риска атеросклероза и связанных с ним КВН у пациентов СКВ. Материал и методы. Обследовано 99 пациентов, средний возраст - 35,9± 10,8 лет (М±ст). длительность заболевания - 106±115,5 мес. Анализировались классические факторы риска. Риск коронарных осложнений оценивался по "Схеме определения индивидуального суммарного (общего) риска клинических проявлений ИБС". Проводилось дуплексное сканирование общих сонных артерий. С-реактивный белок определялся высокочувствительным иммуноферментным методом с использованием коммерческих наборов фирмы Bender MedSystems. Результаты. У 92% пациентов СКВ встречался хотя бы один классический фактор риска атеросклероза (чаще всего ДЛП и АГ). Субклинические проявления атеросклероза, характеризующиеся повышением толщины комплекса интима-медиа, и ДЛП чаше определялись при АГ. чем без нее (р=0,001 и р=0.008 соответственно). Суммарный риск клинических проявлений ИБС в группе с признаками атеросклероза был выше, по сравнению с группой пациентов без атеросклероза (р=0,0001). Обнаружена положительная корреляция между суммарным риском коронарных осложнений и концентрацией СРБ (г=0,27; р=0,02). Заключение. Классические и некоторые "суррогатные" факторы риска имеют существенное значение в прогрессировании атеросклероза и связанных с ним КВН. Для изучения механизмов развития раннего атеросклероза необходим многофакторный анализ различных показателей, изучение их роли в развитии КВН у пациентов СКВ.
